Bijlage 1: Behandelingen die we niet vergoeden

Uitgesloten van het contract is medisch specialistische hulp die niet conform de Nederlandse Zorgverzekeringswet (Zvw) is. Een van de criteria uit de Zvw is dat de zorg voldoet aan de stand van de wetenschap en praktijk. Zorg waarvoor (nog) onvoldoende wetenschappelijk bewijs is dat deze effectief en veilig is (op korte en lange termijn), komt niet voor vergoeding in aanmerking en valt dus niet onder het contract. Hieronder staan een aantal voorbeelden van behandelingen die niet aan bovenstaand criterium voldoen en dus niet onder het contract vallen. Deze lijst is overigens niet limitatief. Ook andere, meer experimentele of innovatieve behandelingen voldoen mogelijk niet aan bovengenoemd criterium. Orthopedie/neurochirurgie

  • Discusprothese;

  • Dynamische stabilisatie door middel van bijvoorbeeld Dynesis, X-stop, Coflex, DIAM-systeem;

  • Minimaal invasieve chirurgie voor een lumbale wervelkanaalstenose;

  • Nesovic-operatie, al dan niet gecombineerd met adductorenrelease, bij sportgerelateerde liesklachten (chronische pubalgia);

  • Minimaal invasieve behandeling van een hallux valgus;

  • Plaatsing totale duimbasisprothese (CMC1-prothese) bij carpometacarpale (CMC) 1-artrose;

  • Vertebroplastiek en ballonkyphoplastiek bij pijnlijke compressie door wervelfracturen;

  • Femoro-acetabulaire chirurgie ter behandeling van FAI (femoro-acetabulaire impingement);

  • Het plaatsen van een Metaal-op-Metaal heupprothese;

  • Kniedistractie bij volwassen patiënten jonger dan 65 jaar met end-stage knieartrose

Injecties en radiofrequente denervatie bij klachten aan het bewegingsapparaat

  • Hyaluronzuurinjecties in een gewricht;

  • Platelet Rich Plasma (PRP) bij laterale epicondylaire tendinopathieën;

  • Facetgewrichtinfiltraties bij chronische lage rugklachten;

  • Epidurale/peridurale injecties bij chronische, aspecifieke lage rugklachten;

  • Radiofrequentie denervatie bij chronische rugklachten.

ESWT en Radiotherapie

  • Extracorporeal Shock Wave Therapy (ESWT) bij schouderklachten;

  • Extracorporeal Shock Wave Therapy (ESWT) bij een fasciitis plantaris;

  • Extracorporeal Shock Wave Therapy (ESWT) bij achillespees tendinopathie;

  • Radiotherapie bij Ledderhose;

  • Radiotherapie bij M. Dupuytren;

  • Radiotherapie bij Achillodynie;

  • Radiotherapie bij Epicondylitis.

Oogheelkunde

  • Intracorneale ringsegmenten bij keratoconus;

  • Epithelial-on collageen crosslinking bij keratoconus en keratectasie;

  • Accomoderende lenzen bij cataract;

  • Femtosecond laser assisted cataract surgery (FLACS).

Neurostimulatie/magnetische stimulatie/elektrische stimulatie

  • Nervus vagus stimulatie bij depressie;

  • Repetitieve transcraniële magnetische stimulatie (rTMS), transcraniële directie currente stimulatie (tDCS), elektrische (corticale) stimulatie en andere vormen van neuromodulatie bij tinnitus;

  • Sacrale neurostimulatie (SNS) bij kinderen en volwassenen met therapieresistente functionele obstipatie;

  • Dorsal Root Ganglion Stimulation bij Failed Back Surgery Syndrome (FBSS);

  • Intrathecal Drug Delivery (ITDD) bij Failed Back Surgery Syndrome (FBSS);

  • Dorsal Root Ganglion Stimulation bij diabetische neuropathie of dunne vezelneuropathie;

  • Spinal Cord Stimulation en Dorsal Root Ganglion Stimulation bij postherpetische pijn;

  • Spinal Cord Stimulation en Dorsal Root Ganglion Stimulation bij neuropathische pijn door letsel;

  • Spinal Cord Stimulation bij angina pectoris;

  • Spinal Cord Stimulation bij perifeer arterieel vaatlijden/ischemische pijn;

  • Deep Brain Stimulation als behandeling van therapieresistente depressie en therapieresistente Gilles de la Tourette;

  • Transcraniële magnetische stimulatie bij depressie;

  • Elektrische stimulatie van de lagere oesofageale sfincter bij gastro-oesofageale refluxziekte.

Ablatie/Embolisatie, zoals TACE, SIRT

  • Radiofrequente ablatie (RFA), Laser-induced interstitële thermotherapie (LITT) en/of Transarteriële chemo-embolisatie (TACE) bij levermetastasen (uitgezonderd TACE als palliatieve behandeling in salvagesetting van niet-reserceerbare functionerende neuroendocriene levermetastasen);

  • Thermale ablatie bij resectabele colorectale levermetastasen;

  • Transarteriële chemo-embolisatie (TACE) bij intrahepatisch cholangio(cellulair) carcinoom;

  • SIRT/radio-embolisatie bij levermetastasen;

  • SIRT/radio-embolisatie bij intrahepatisch cholangio(cellulair) carcinoom;

  • Percutane leverperfusie met melfalan bij leverdominante, niet-resectabele mestastasen van een uveamelanoom.

Transplantaties

  • Autologe hematopoietische stamceltransplantatie bij therapierefractaire Morbus Crohn;

  • Transplantatie eilandjes van Langerhans;

  • Uitname, cryopreservatie en transplantatie van gecryopreserveerd ovariumweefsel voor presevatie van ovariële functie en fertiliteit bij gonadotoxische behandelingen.

Urologie

  • PCA3-test bij prostaatkanker;

  • HIFU bij prostaatkanker;

  • Renale denervatie bij therapieresistente hypertensie;

  • Baroreflex activatietherapie bij therapieresistente essentiële hypertensie.

KNO/tinnitus

  • Cochleair implantaat bij tinnitus;

  • Microvasculaire decompressie bij tinnitus;

  • Repetitieve transcraniële magnetische stimulatie (rTMS), transcraniële directie currente stimulatie (tDCS), elektrische (corticale) stimulatie en andere vormen van neuromodulatie bij tinnitus.

Chirurgie/Gastro-enterologie

  • Laparoscopische adhesiolyse bij chronische buikpijn;

  • Duodenal-Jejunal bypass (EndoBarrier) voor de behandeling van obesitas met of zonder diabetes mellitus type 2;

  • Bariatrische chirurgie bij mensen met diabetes mellitus type 2 en een BMI van minder dan 35 kg/m2;

  • Endoluminale fundoplicatie met Esophyx® bij gastro-oesofageale reflux;

  • Wormtherapie bij inflammatoire darmziekten.

Longgeneeskunde/Cardiologie/Interne geneeskunde

  • Occlusie van het linkerhartoor bij atriumfibrilleren;

  • TAVI bij patiënten met symptomatische ernstige aortaklepstenose en een laag of gemiddeld operatierisico;

  • Langdurige behandeling met antibiotica bij patiënten met post-treatment lyme disease syndrome (PTLDS);

  • Mannitol-infusie bij een complex regionaal pijnsyndroom type 1.

Overige

  • Assisted hatching bij IVF;

  • Steriele waterinjecties (SWI) ter pijnbestrijding bij een bevalling;

  • Foamsclerosering bij stamvarices;

  • Dendritische celvaccinatie bij de behandeling van patiënten met stadium IIIB en IIIC melanoom na complete resectie;

  • Veneuze angioplastiek bij chronische cerebrospinale veneuze insufficiëntie (CCSVI) bij multiple sclerose (MS);

  • Klinische opname voor kinderen met ernstige obesitas. Het gaat hier om de gecombineerde leefstijlinterventie met opname met als doel blijvende gewichtsreductie en gedragsverandering;

  • Vroege intensieve neurorevalidatie bij volwassenen met niet-responsief waaksyndroom of laagbewuste toestand.
